As the parent/legal guardian of the student listed below I hereby give my permission for him/her to accompany the class on physical education outings/field trips/other educational outings off the EMA school grounds. I understand that trips may occur throughout the school year and that all such outings will be under the supervision of a teacher or staff member.

EMA will send written notice least 1 day in advance notifying you that your student will be out of the building on an area field trip. On occasion, an impromptu trip may take place and a telephone call will be made for those situations.

In consideration of the benefits to be derived, any and all claims against Evelyn Mack Academy or its agents with regard to accident, illness, injury, damage or other loss incurred or suffered by my child are hereby expressly waived.
